Do rules need approval, and when does a rule apply?

No.

A rule needs no approval, and since version 13.2.49.79 a plan does not either: it applies from the moment someone saves it until someone changes or removes it, and the panel of a rule says Always applies ยท no approval needed. Rules have no thumbs-up: unlike plans, they are not voted, because every rule binds.

From then on every agent working on the project, for every team member and from every PC, receives the rule before it changes anything and must follow it; an agent already at work receives it before its next write. A rule is what a person wrote: since version 13.2.49.74 there are no evidence levels such as Measured, Declared, Inferred or Unknown, and SyncNow neither measures nor verifies what a rule says.

Because a rule binds every agent at once, write it as clearly as the description of a plan: what must or must not be done, where, and why. To change a rule, edit it: agents follow the new text from then on. See How do my agents use the Rules before they change my site?.
